You are doing one of the most important things you can: Reaching out. Keep on reaching out (to physicians, sites, resource services, family and friends).    

A key word to always remember when with her is "patience." Sadly, I initially showed a good amount of frustration with Mom, about the very things she couldn't help. Finally I learned - duh - she responded to my frustration. (Those with dementia, no matter how bad, respond to the tone of frustration in a voice; likewise, they respond to caring in a voice.) Both our lives changed for the better when I continued to proactively nurture a positive environment of respect and love. (I worked out my frustrations and fears away from her.) 

While planning for what could be next, try to live in the moment with her, finding moments of joy and comfort in each time you share.

Review "dementia" references on this site, including those from MindingYourElders. Add  "care giver" and "caregiver" searches. Reach out to those who say things that resonate. 

It is my understanding that how dementia progresses varies by person, and cause. Mom's was a secondary condition to a primary issue of mini strokes, then she had a big stroke. (Sometimes the minis lead to a big one, sometimes not.)
